OMPH Intercampus Registration
for All Oregon State University Students
For online classes offered by the School of Nursing at OHSU
IMPORTANT: Registration must be completed no later than two weeks prior to the date classes begin in order to set up your access to Sakai and the course. REGISTRATION PROCEDURE FOR ALL OMPH STUDENTS:
Biostatistics, Environment, Safety & Health; Epidemiology, Health Management & Policy; Health Promotion, International Health Note: For OHSU Library off-campus access, see https://www.ohsu.edu/library/libcdapp.shtml. Other information about library policies is available at www.ohsu.edu/library/circpol.shtml and www.ohsu.edu/library/offcampus/distintro.shtml. Note: For your online course, you need an e-mail provider that easily sends and receives attachments. Check with your home institution for an e-mail account. If you want an OHSU e-mail account, go to http://www.ohsu.edu/son/academic/app-nds.shtml which takes you to the “Non-degree Status” page on the OHSU School of Nursing website. Go down to number 4 (4.) and follow the instructions.
For OMPH classes at OHSU School of Medicine:
1. Obtain the OSU Joint Registration Form from http://www.hhs.oregonstate.edu/ph/information-graduate-students-new-and-returning
2. Contact the PHPM Administrative Assistant, Natalie Chin, to make sure there is room in the course. You will also need to obtain permission via email from the instructor directly.
3. If there is room in the course and you have obtained permission, please forward the completed enrollment form via fax at 503-494-4981 or as a PDF attachment.and the instructor's email documenting permission to Natalie who will secure the final departmental approval signature and fax the form to the OSU Registrar on your behalf.
4. The OSU Registrar will send grade sheets to the PSU department at the end of the term. The instructor will return the form to the OSU Registrar's Office for recording.
For OMPH classes at PSU School of Community Health:
1. Obtain the OSU Joint Registration Form from http://www.hhs.oregonstate.edu/ph/information-graduate-students-new-and-returning.
2. Email Elizbeth (Beth) Bull at bulle@pdx.edu to seek confirmation that space is available in the class and that the proposed registration is acceptable. Program staff will contact the instructor on your behalf to obtain permission.
3. If permission is granted, email the Joint Campus Registration Form as a PDF to Beth Bull, bulle@pdx.edu or fax at 503-725-5100. (If sending a fax, email Beth to notify her that it is sent. Emailing PDF's is preferred.) Beth will complete the registration form and submit it to the OSU registrar.
4. The OSU Registrar will send grade sheets to the PSU department at the end of the term. The instructor will return the form to the OSU Registrar's Office for recording.
For OMPH classes at PSU Division of Public Administration:
1. Obtain the OSU Joint Registration Form.
2. Email the PSU instructor of record to seek confirmation that space is available in the class and that the proposed registration is acceptable.
3. If permission is granted, email the Joint Campus Registration Form as a PDF to Becky Fidler<rfidler@pdx.edu> or fax at 503-725-8250. (If sending a fax, email Becky to notify her that it is sent. Emailing PDF's is preferred.) Becky will complete the registration form and submit it to the OSU registrar.
4. The OSU Registrar will send grade sheets to the PSU department at the end of the term. The instructor will return the form to the OSU Registrar's Office for recording.
Profiles
The Graduate Certificate in Public Health (GCPH), offered through OHSU School of Nursing, is now accepting applications for admission Winter 2012 and Fall 2012. The GCPH is offered online. Information about the GCPH can be found here.
